
Pathways to Community is a collaborative partnership between the Coconino County Sheriff’s Office, Coconino County Health and Human Services (CCHHS), and many other partner agencies, providing re-entry support services for individuals leaving the Coconino County Jail. Individuals being released from custody receive immediate connection to social, behavioral, housing, transportation, and legal services.
This program maximizes the availability of resources for individuals upon their release and for up to 72 hours following release, improving their ability to re-enter our community successfully.
For our most vulnerable residents, access to substance abuse treatment and prevention services, mental health services, education, employment support, housing services, transportation, social services, and healthcare are key to reintegrating in community.
Diversion programs such as Pathways to Community can drastically improve our residents' wellbeing, and ultimately break the cycle of violence, substance abuse, and criminal activity that can impact generations. Pathways to Community services provide access to needed resources, and in turn, a path to avoid incarceration and to improve lives and overall wellbeing.
