Serving individuals age 12 and over. Minors age 12-17 years old must be accompanied by a legal guardian and a consent form signed. Pfizer COVID-19 vaccine available.
This indoor site offers vaccinations as follows:
Open Wednesday - Saturday, 10:00 am - 6:00 pm. (Closed Sunday, Monday and Tuesday)
May 30 - 31 and June 1: Closed for Memorial Day Holiday

Parking is provided at no cost in front of the NAU Fieldhouse and overflow parking available in the parking garage.

Patients who received their first dose of Pfizer vaccine at another location are eligible to receive their second dose at the NAU site and must bring their CDC vaccination record card.

Appointments are at www.podvaccine.azdhs.gov or by calling 1-844-542-8201. Appointments are not required.
